Four Steps to Enrollment - Current 20/21 School Year

1. Locate	Use the school locator to confirm which school your child will attend.
2. Apply	Create an account and submit an application. Allow 15 minutes to finish the form for each new student.
3. Complete Forms and Deliver Documents	Print the forms listed below. Fill them out and bring them along with the child's original birth certificate to the enrolling school. Pre-K Students Only Lechner Early Education Program Registration Form 1st-8th Grade Students New to District 39 Only Student Records Request with Fax and Phone
4. Pay Fees	Fees will be assessed after steps 2 and 3 are completed.

The following forms will be required prior to starting in school:

• Child's original birth certificate
• Two original documents that provide proof of residency (details on the Affirmation of Residency Form)
• Child Health Exam
• Dental Exam
• Eye Exam Report
• Health Letter 20-21 Enrollment

Not Required: McKinney-Vento eligible children are those who lack a fixed, regular and adequate nighttime abode. In general, children or youth living in motels, transitional housing, shelters, the street, cars, abandoned buildings, and other inadequate accommodations may be considered eligible for McKinney-Vento services.
